Northern Ireland Planned Healthcare Scheme
Have your skin lesion treated in the North — and claim the cost back
If you live in the Republic of Ireland, you can have a skin biopsy or lesion removed privately in Northern Ireland and apply to the HSE for repayment afterwards. You do not need to be on a waiting list to qualify.

In plain English
One scheme, one simple idea: you pay, then you're repaid
The Northern Ireland Planned Healthcare Scheme (NIPHS) lets people living in the Republic of Ireland access planned, private healthcare in Northern Ireland — and reclaim the cost from the HSE. It replaced the cross-border arrangement many people already know. You pay The Skin Cancer Institute for your treatment, then apply to the HSE for repayment towards what you paid.
- 01
You pay for your care
You settle the cost of your skin biopsy or lesion removal with The Skin Cancer Institute, with pricing explained clearly upfront.
- 02
You submit your claim
You complete the HSE healthcare-costs application form and send it in with your invoice and receipt. Our team helps you prepare it.
- 03
The HSE repays you
The HSE repays you the lower of what you paid in the North or the matching public price in the Republic — up to the amounts shown below.
What you can claim back
The three skin procedures patients claim most
The HSE works out your repayment using Diagnosis-Related Group (DRG) codes — a single code that covers your whole case. These are the codes and maximum day-case repayments most relevant to skin cancer and skin lesion care.
Skin biopsy · non-malignant
J67BBiopsy of a non-cancerous skin condition
MINOR SKIN DISORDER, MINC
Skin biopsy alone, for a non-malignant skin condition
Excludes excisional biopsy — see excision card
Repays up to
€668
Skin biopsy · malignant
J69BBiopsy of a cancerous skin condition
SKIN MALIGNANT, MINC
Biopsy alone, for a malignant skin condition
Excludes excisional biopsy — see excision card
Repays up to
€604
Excision of lesion · all sites
J11BRemoval of a skin lesion
OTH SKN, SUBC TIS & BRST INTERVTN, MINC
Excision of a skin lesion, malignant or non-malignant
Covers all body sites
Repays up to
€829
A consultation can be claimed too. Outpatient appointments — including a consultation, a diagnostic test or a minor procedure on the same day — are repaid up to €194 for appointments from 1 May 2023. If you have more than one appointment on the same day, the €194 maximum still applies once. The HSE confirms the exact figures and the final DRG code after your treatment, based on your medical notes.

Estimated Maximum Repayment

An estimate only. The HSE repays whichever is lower — the cost in the North or the public price in the Republic — and confirms the final code and amount from your medical notes. Prices are reviewed each year.
Your patient journey
From referral to repayment, step by step
Skin biopsies and lesion removals are day-case procedures, so the path is short. Here's what happens from start to finish.
- 01
Get a referral
Ask your GP — or a consultant you see as a public patient — for a referral letter for the treatment you need. A referral obtained through private health insurance can't be used for the scheme.
- 02
Attend your consultation
You meet your Skin Cancer Institute consultant to assess the lesion and plan your care. For day cases this can be in person, by phone or by video.
- 03
Have your procedure
Your biopsy or excision is carried out as a day case — no overnight stay. Many skin lesions are assessed and treated in a single visit.
- 04
Pay and keep your paperwork
You pay for your treatment and keep your itemised invoice and receipt. These are what you'll submit to the HSE with your claim.
- 05
Apply to the HSE
Complete the NIPHS healthcare-costs application form and send it with your invoice and receipt. Our team helps you get it right the first time.
- 06
Receive your repayment
The HSE repays you towards the cost, up to the relevant DRG amount. The repayment is made to you, the patient.
Day case or overnight? Day-case skin procedures don't need approval before you travel. Only treatments needing an overnight hospital stay (inpatient care) require prior notification (opens in a new tab) from the HSE first. You can also apply for prior notification voluntarily to get an indication of what you'll be repaid.
Who can apply
You may qualify if…
- You live in the Republic of Ireland — or intend to live here for at least one year.
- You'd be eligible for this care in the public health system in the Republic.
- You have a referral from a GP, or a consultant you see as a public patient.
- You have an outpatient consultation before any day-case treatment.
- You do not need to be on a waiting list to qualify.
A note on private health insurance
Having private health insurance does not exclude you from the scheme. However, you cannot use a private-insurance referral to access care under the NIPHS — your referral must come from a GP or from a consultant you're seeing as a public patient. If you're unsure whether you qualify, the HSE can help, or speak to our team and we'll talk it through.

Cross-border skin care, explained
Common questions about the NIPHS and claiming back from the HSE
Do I need to be on a waiting list to use the scheme?
No. The NIPHS is open to anyone who would be entitled to the same treatment in an Irish public hospital. You don't need to be on a waiting list — you only need to be eligible for the care and to have a valid referral.
How much will I actually be repaid?
You're repaid whichever is the smaller amount: the cost of your treatment in Northern Ireland, or the matching public price in the Republic of Ireland (the DRG amount). So if your procedure in the North costs less than the DRG figure, you're repaid what you paid; if it costs more, you're repaid up to the DRG amount.
Do I need approval before I travel for a skin procedure?
For day-case skin procedures — biopsies and lesion removals with no overnight stay — you do not need prior approval. Only treatments requiring an overnight hospital stay need prior notification from the HSE beforehand. You can still apply for prior notification voluntarily to get an indication of your repayment.
Can I claim for my consultation as well as the procedure?
Outpatient appointments are repaid up to €194 for appointments from 1 May 2023. If you have several appointments on the same day — for example a consultation and a minor procedure — the single €194 maximum applies. The HSE confirms the exact figures based on your records.
I have private health insurance — can I still apply?
Yes. Having private health insurance doesn't exclude you. The one condition is that you can't use a private-insurance referral to access care under the scheme — your referral must come from a GP or from a consultant you see as a public patient.
What does the scheme not cover?
The NIPHS covers the cost of your healthcare only. It does not cover travel, accommodation or other related costs. It also can't be used for treatment in a public healthcare service in Northern Ireland — it applies to planned, private care.
